TYPES OF CLAIM

MEMORIAL Plan (Traditional, Fixed, Interment, Cremation, Colossian, Corinthian, Titus)
· Plan Termination
· Unrendered Service Benefit

Requirements:

a. Original Policy Agreement with Insurance Certificate

b. Original Certificate of Full Payment

c. Duly accomplished Acknowledgement Slip (AS)

d. Two (2) valid IDs of the planholder with photocopy, valid IDs are:
· Company ID
· Driver’s License
· Laminated GSIS / HDMF / Philhealth
· OWWA
· Unexpired Passport / Seaman’s Book
· PRC License
· Postal ID
· Senior Citizen ID
· Voter’s ID

e. NSO Birth Certificate or Marriage Contract – in case of change in name or status

f. Original / Certified true copy of Death Certificate (if plan holder deceased) – to be presented by beneficiary/ies

g. Authorization Letter from planholder (FOR FILING ONLY) – if to be filed through a representative and:

o Photocopy of two (2) valid IDs of representative (original to be presented)

o Photocopy of two (2) valid IDs of plan holder

h. If check is to be– claimed by a representative, original and duly notarized Special Power of Attorney (SPA) is required; if planholder is abroad, SPA should be authenticated by the Philippine Consul’s Office in the country where the planholder is working or residing

CHECKLIST OF DEATH CLAIM REQUIREMENTS:

  • FOR THE DECEASED PLANHOLDER
  • FOR THE BENEFICIARY / CLAIMANT
  • DISABILITY CLAIM
· ORIGINAL INSURANCE CERTIFICATE; IF LOST, SUBMIT A NOTARIZED AFFIDAVIT OF LOSS

· PLAN AGREEMENT AND CERTIFICATE OF FULL PAYMENT – 2 xerox copies

· DEATH CERTIFICATE
   · 1 certified true copy (original, with dry seal or documentary stamp from the NSO or the LCR)
   · 1 photocopy
   · If planholder died abroad, death certificate should be original and certified / authenticated by the Philippine Consul’s Office

· BIRTH CERTIFICATE
   · 2 xerox copies either from NSO or the Local Civil Registrar (clear copy)
   · If Negative Birth Certificate, original copy must be submitted, supported by baptismal certificate, voter’s registration record and/or SSS ID (with birthdate of deceased planholder)

· IDs AND PUBLIC DOCUMENTS WITH SIGNATURE OF DECEASED – 2 xerox copies

· DULY REGISTERED MARRIAGE CERTIFICATE (If applicable) – 2 xerox copies

· ORIGINAL, NOTARIZED AND SEALED JOINT AFFIDAVIT OF 2 DISINTERESTED PERSONS - if there is discrepancy in the planholder’s personal data vs. the APEC plan data
· CLAIMANT’S STATEMENT – 2 copies; form to be provided by APEC

· BIRTH CERTIFICATE
   · 2 xerox copies either from NSO or the Local Civil Registrar (clear copy)
   · If Negative Birth Certificate, original copy must be submitted, supported by baptismal certificate, voter’s registration record and/or SSS ID (with birthdate of the beneficiary/ies)

· DULY REGISTERED MARRIAGE CERTIFICATE (If applicable) – 2 xerox copies

· VALID IDs – 2 xerox copies

· ORIGINAL, NOTARIZED AND SEALED JOINT AFFIDAVIT OF 2 DISINTERESTED PERSONS - if there is discrepancy in the claimant’s personal data vs. the APEC plan data vs. IDs (or submit baptismal cert., marriage contract, voter’s affidavit)

ADDITIONAL REQUIREMENTS - AS MAY BE APPLICABLE – Submit 1 original and 1 xerox copy

· Physician’s Statement duly notarized and Certified true copy of Hospital records with ADMITTING history and Physical Examination – for contestable plan only

· FOR MINOR BENEFICIARY WITH INSURANCE BENEFIT OF LESS THAN P50,000
   · Affidavit of Guardianship to be executed by the Natural Parents

· FOR MINOR BENEFICIARY WITH INSURANCE BENEFIT OF P50,000 UP TO P150,000 – Natural or Surviving Parent should submit the following:
   · Letter-request for the release of the insurance benefit
   · Notarized Deed of Undertaking
   · Affidavit of Guardianship

· FOR MINOR BENEFICIARY WITH INSURANCE BENEFIT OF MORE THAN P150,000 (Parent is the guardian)
Guardian should file with the RTC a Petition for approval of the bond under Art. 225 of the Family Code then submit the ff.
   · Petition
   · Decision
   · Oath of Office
   · Proof of Bond and Court Approval of the Bond

· FOR MINOR BENEFICIARY WITH INSURANCE BENEFIT OF MORE THAN P150,000 (Parent is NOT the guardian)
   · Order of the Court appointing the Person as Guardian of the Minor
   · Letters of Guardianship issued by the Court
   · Oath of Office
   · Proof of Bond and Court Approval of the Bond

· IF DEATH IS DUE TO ACCIDENT
   · Certified True Copy of Police Investigation Report, Autopsy Report and Progress Report
   · Affidavit of Eyewitness/es regarding the Incident
   · Traffic incident report with sketch (if death is due to vehicular accident)

· IF THERE IS NO DESIGNATED BENEFICIARY OR THE DESIGNATED BENEFICIARY DIED AHEAD OF THE PLANHOLDER
   · Original and Notarized Affidavit of Extra Judicial Settlement/Affidavit of Self-Adjudication – Pls. include the phrase "I/We are the only compulsory surviving legal heir OF THE DECEASED PLANHOLDER"
   · Certified true copy of the death certificate of beneficiary
   · Birth Certificate, Marriage Certificate and IDs of those mentioned in the Affidavit

· IF THE PLANHOLDER AND THE BENEFICIARY ARE BOTH DECEASED AT THE TIME OF FILING OF CLAIM, PLANHOLDER DIED AHEAD OF THE BENEFICIARY
   · Original and Notarized Affidavit of Extra Judicial Settlement/Affidavit of Self-Adjudication – Pls. include the phrase "I/We are the only compulsory surviving legal heir OF THE DECEASED BENEFICIARY"
   · Certified true copy of the death certificate of beneficiary
   · Birth Certificate, Marriage Certificate and IDs of those mentioned in the Affidavit

· IF THE DESIGNATED BENEFICIARY IS A COMMON-LAW SPOUSE
   · Original copy of the CENOMAR of insured from NSO
   · Affidavit regarding marital status of Planholder and the beneficiary

· COURT DECLARATION (PRESUMPTION OF DEATH) – FOR MISSING BODY
·FOR UNRENDERED FIXED VALUE PLAN
   · Original policy
   · Original Certificate of Full Payment
   · Letter of intent from the primary beneficiary regarding claim for the unrendered service benefit (please Indicate reason why plan was not used for service)
   · Certified True Copy of Death Certificate (with dry seal)
   · IDs of primary beneficiary
   · Marriage Certificate, if primary beneficiary is married
   · Insured’s Statement (2 copies each)
   · Physician’s Statement (2 copies each)
   · Certified true copy of Hospital Records with Admitting History and Physical Examination (2 copies each)
